Healthcare Savings Breakthrough
with New Hospital-to-Home Rehab Program

OUR FACILITY: Although our on-site physical therapy allows us to operate as a Rehabilitation Center we are State-licensed as a level 3 adult foster home. This foster home designation allows us to provide 24-hour care and to perform medication management with those residents who require it. The home occupies 5,000 sq feet and is situated on one acre of land. The front of the home overlooks the breath-taking Elkhorn Mountain range.

 

LOCATION: The facility is located at 3490 10th Street in Baker City, Oregon, less than a mile from St. Elizabeth’s Hospital. Although we are located in Eastern Oregon we accept qualified residents from any state.

OUR FOCUS: To provide short-term rehab services (including physician-prescribed physical therapy) for residents desiring quick recovery from surgery, illness, or injury. Our mission is to help individuals regain strength and function in order to return to their homes. Although we seek residents with strong rehab potential and a desire to return home we consider long-term care residents on a limited basis. Everyone is screened to ensure a good match.

SETTING: We offer residential care in a home-like environment with home-cooked meals, an excellent staff-to-patient ratio and an on-site physical therapist (Tom LeBlanc PT).

RECENT ADDITION: We now feature an indoors Endless Pool ™ which we use for doctor-specified exercises and as a recreational activity for our residents. Maintained at 92 degrees, our therapy pool has proven valuable in helping residents regain strength, balance and range of motion. Therapeutic uses include: water exercises, muscle stretching, water aerobics, gait training and balance training.


SAVINGS! In some cases we are able provide more than twice the rehab services at half the price of the average non-skilled nursing home. (See Press Release for important details). This equates to a shorter stay and reduced out-of-pocket expenses.

Medicare provides coverage for qualified patients undergoing treatment in hospitals, swing bed programs and skilled nursing facilities. Medicare DOES NOT provide coverage to patients in assisted living facilities, adult foster homes and non-skilled nursing homes. Individuals are encouraged to remain in facilities where coverage is available as long as possible. However, if the Medicare coverage ends before a patient is ready to return home the cost of services can increase dramatically. This is where we are able to step in and reduce those out-of-pocket expenses.

ABOUT THE OWNERS: Transition Home is owned and operated by Tom and Donna LeBlanc. Tom has practiced physical therapy, for over 30 years, in a variety of rehab settings. These include 17 years in private practice and numerous years in long-term care, home health, acute hospital, and hospital swing bed settings. Donna has been a compassionate caregiver for over 20 years.
