A Rehabilitative Adult Foster Home

  • Individualized exercise and training programs
    Individual physician-ordered exercise programs designed by Tom LeBlanc, a licensed physical therapist, are custom-tailored to Transition Home residents, on a case-by-case basis, to restore muscle strength, mobility, balance and ambulatory skills in the shortest time possible. When this is done close communication will be maintained with the resident’s physician.
  • Appropriate therapy
    Transition Home provides physical rehabilitation based on individual needs, potential and abilities to improve range of motion, motor skills and coordination. All skilled physical therapy is ordered by the attending physician.
  • Training in activities of daily living
    Transition Home helps its clients to maximize their abilities, independence and quality of life so they can safely return to their own homes and resume living independently. Activities of Daily Living (ADL’s) include dressing, grooming and personal care.
  • Monitoring of vital signs as needed
    Each client’s vital signs (blood pressure, heart rate, temperature and blood oxygen level) are checked periodically and as needed to ensure they’ve not deviated from established baselines.
  • Medications administration
    Medication management is provided under our adult foster home license. Trained caregivers monitor and administer prescription medications to those clients who need them, ensuring they get the right dosage at the right time.
  • Healthful diet of home-cooked meals
    Home-style meals maintain an artful balance between nutritional needs and medical limitations, while taking clients’ likes and dislikes into consideration.
  • Training in the use of specialized adaptive equipment
    Sometimes specialized equipment is needed to ensure independent living. We have access to a large variety of adaptive devices that we train residents how to use.
  • Residents’ attending physicians kept informed
    When physicians request physical therapy they will be kept informed, in writing, of the condition and progress of their residents. This includes an initial physical therapy evaluation and 30-day status reports.
  • Consultations with RNs are available
    Registered nurses are available for consultation. Oregon adult foster home laws allow RN’s to “deligate” certain tasks. When this is done the RN trains our care givers in certain tasks to be carried out between regular RN visits.
  • Occupational therapy and speech therapy available on home health basis
    When occupational therapy or speech therapy are needed to enhance the rehabilitation efforts they can be utilized by licensed therapists.
  • Progress charts and reports
    Extensive documentation is maintained on the residents in order to ascertain progress in their specialized training programs.
  • Less than 1 mile from St. Elizabeth’s Hospital
    When we searched for the ideal facility location we selected a location less than a mile from the hospital. This allows rapid response time in the event of medical emergencies.
  • Free cable TV
    Each resident can plug his TV into cable to allow for viewing of their favorite programs.
  • Free long distance phone calls
    Although each resident is allowed to connect his own private phone line, we offer free long distance calling.
  • Free information and referral services from the Transition Home Web site
    We are striving to be a community resource of helpful information that relates to health and independent living. This growing list of articles appears in our Rehab Topics section.
