We are located in historic Baker City, Oregon, overlooked by several picturesque mountain ranges.

While seeking the “Perfect Location” for our proposed Rehabilitative Adult Foster Home we considered the following:

1. Close proximity to the hospital. We selected a home less than a mile from St. Elizabeth’s Hospital.
2. Sufficient floor space on the entry level. Transition Home occupies nearly 3,000 feet on the entry level. It consists of four large bedrooms and three bathrooms.
3. Large fenced yard to allow for family style picnics, barbecues and other outdoor activities. The property occupies about an acre of land. Several large shade trees and a number of cherry and apple trees provide a park-like setting.
4. Activities for visiting grandchildren. We have more than enough toys and activities to make the visit appealing to our residents’ grandchildren.
5. Garden space to allow residents to plant flowers or vegetables, if they so desire.
