with New Hospital-to-Home Rehab Program

Oct 25, 2004, Baker City, Oregon – Transition Home announces a new transitional care (hospital-to-home) program that provides rehabilitation that can save patients thousands of dollars and return them to their homes in record time. The program makes available doctor-prescribed physical therapy 2-3 times per day versus the 2-3 times per week typically offered in non-skilled nursing homes.

Individuals recovering from illnesses, injuries or surgeries sometimes need additional rehab when they no longer qualify for acute hospital and swing bed programs. Patients without Medicare or those whom Medicare no longer covers must decide where to continue their recovery until they are able to return home. Baker City’s Transition Home (a State-licensed level 3 adult foster home) is an option for these kinds of patients.
